What we need

In every case of Personal Injury and Clinical Negligence:
	A letter of instruction that includes a specific agreement to accept the Consultant's Terms & Conditions and which outlines the nature of the case and requirements of the report ,
	Copies of any reports in the case compiled by other experts,
	Copies of GP medical records, including computer-held records, computer-generated prescribing date, handwritten notes and correspondence,
	Mental health notes (if any).

Depending on the nature of the case it may be necessary also to see occupational health records, personnel records, the Claimant's statement, statements from other witnesses and so on.
